If you would like a third party (wife, husband, child, friend etc) to have access to any information on your medical records, you will need to complete an Information Sharing Consent Form. By signing this form you are showing you are happy for us to discuss test results, arrange appointments with, and/or relay messages from the GPs or our admin team with the person you nominate.
A member of staff will need to witness you signing the form and once authorised by a GP, it will be added to your medical notes.
