Contact Us Online

For queries or help regarding an existing medical condition or admin requests, please can we ask that you complete a Systmconnect form online as the GP’s do not take questions via our surgery email address. This would also include requests for sick notes.

To access the service please click here. You can also submit the form via the NHS app.

Please note this is not for urgent queries or new medical conditions. If you think you need to be seen face to face, the quickest way to book an appointment is to ring the surgery.

Telephone

Our surgery telephone number is 01258 880210.

For urgent on the day appointments, please call at 8.30am or 2.00pm to book. For all non urgent matters, please can we ask you call after 9.30am.

Please note that calls may be recorded.
