Before filling out a registration form, please check you are in our catchment area.

Please note that the boundary shown in the image is not exact. Our admin staff will confirm if you are within our area when your registration form is received. For online registrations, once our admin team verifies you are within our catchment area, we will send you an acceptance message either via email or text message.

To register

We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. The service is designed and run by the NHS, so your personal information is safe and makes it easier for you to register.

Paper forms are still available if you need one. Please ask for a Registration Pack at reception which includes a medical questionnaire.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Temporary Residents

If you are temporarily living in the area (for up to 3 months), are more than an hour away from your current GP surgery, and need medical treatment you can apply to register as a Temporary Resident, enabling you to take full advantage of all our medical services.

Please do not use the above Registration Form if you are registering as a Temporary Resident. You can either complete the correct form when you arrive at the surgery for your appointment, or reception can take your details over the phone.

Change of Personal Details

If you change your name, address or telephone number, please give full details to reception, including your postcode.  Please remember to tell us if details for other members of your family have also changed.

If you move outside the practice area you will be asked to register with a doctor nearer to your new home.

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.
